Cannot access unlock request form

HI, I've been trying to access the Unlock Code Request Form on this website - is there an issue with this link? It comes up "Federated Login 404" and telling me to try Three.ie when I enter my number to request the form. Thanks, Lorraine

I was able to get through to someone on the live chat who could unlock it for me
