Cancel Mobile broadband and return modem

Hi,

I signed up to Mobile broadband last week. The service is abysmal where I am and I want to cancel and return the modem. I am within the cooling off period. All I want is the case reference number so I can fill the form and return it ASAP. I have been trying for days to contact three through Facebook, Twitter and phone to no avail. Please help.
