Same issue as this user. Pretty useless that nothing in these threads gets posted to indicate how an issue was resolved.

Both mobile app and desktop same issue. "new" account as in haven't topped up online til now. No problems with my payment method. After going through the payment pages just getting the error every time.

and yes, before you ask, it happens the same way on all devices. all browsers. All possible ways I've tried it: yes. Same error.
and the "steps" I'm taking are just filling out all the exact fields required for topup and topup payment by card.

I have checked here for you and I can confirm that our top-up services are currently up and running. As this isn't a network outage in relation to top ups can you try to log out of your My3 app and log back in to try topping up and if this doesn't work can you try calling 1913 or topping up online here to see if any of these platforms work for your account?
